Integrated Physical Therapy Services, Inc. accepts most insurances for physical therapy services. If you do not see your insurance provider below, you may still receive access to our services. Call our office and we’ll be happy to contact your insurance for you to determine coverage, including deductibles and co-pays. Cash payment is also accepted.

Private Insurance

We accept insurance for all Medicare recipients. Patients who have more than one insurance policy must inform us which is primary and complete a Medicare Secondary Payer Form. We’ll provide billing to both Medicare and secondary or supplemental insurances.


Medicaid recipients are covered for physical therapy treatment in Ohio. Contact us for current information on how many appointments are covered per year.

MyCare Ohio

We are a MyCare Ohio participant. MyCare Ohio is a managed care program for Ohioans who receive both Medicare and Medicaid federal health insurances. This program coordinates Medicare and Medicaid services together under one health insurance plan.

Veterans Administration

Veterans and their spouses who receive physical therapy at Integrated Physical Therapy Services are covered by their VA benefits, providing the veteran:

    • Enrolled in VA health care on or before August 1, 2014
    • Enrolls as a recently discharged veteran within 5 years of separation

Also, the veteran must meet one of the following criteria:

    • Needs to wait over 30 days for an appointment at the local VA clinic
    • Enrolls as a recently discharged veteran within 5 years of separation
    • Lives over 40 miles driving distance from the nearest VA health care facility
    • Resides somewhere other than Guam, American Samoa or the Philippines and needs to travel by air, boat or ferry to the nearest VA medical facility
    • Faces an excessive burden in travelling to a VA medical facility based on geographic challenges, environmental factors or a medical condition
    • Resides in a state without a full-service VA medical facility and lives more than 20 miles from such a VA facility

Workers’ Compensation

All workers’ compensation patients must be approved by workers’ compensation prior to beginning physical therapy. Patients whose workers’ compensation claim is denied are responsible for any outstanding physical therapy charges.

Cash Pay

We offer physical therapy services for cash payment for patients who choose not to use or do not have insurance coverage.

Auto Insurance

Auto accident patients are responsible for any physical therapy charges not paid by their auto insurance companies or the auto insurance companies of other parties involved in the accident. We do not file motor vehicle accident (MVA) claims with private health insurance plans.
